How do I initiate hospice care
If you feel you or your loved one would benefit from hospice care, please reach out to your primary care physician to initiate that discussion. If your physician agrees and sees the need for hospice care, they can directly send a referral to Comfort Care Hospice.
If you do not have a physician, you can call our office and our team will be glad to have a visiting physician come to your home and review your medical history. After their evaluation, if they feel hospice is appropriate, they can then send a referral to Comfort Care Hospice to initiate care.
